Gaming forms


About our forms facility

This facility provides access to selected NSW Office of Liquor, Gaming and Racing and Casino, Liquor and Gaming Control Authority forms.

At present there is no capacity to submit forms online as most of the forms require a signature. This means you must download (ie. copying on to your own computer), print the form, and complete it by hand.

However the office is examining ways to provide online services.
